The Pavilion at Creekwood is proud to be the newest healthcare and rehabilitation center in Mansfield, Texas. Our state-of-the-art, 126-bed skilled nursing facility offers both short-term and long-term rehabilitative care.

The Rehabilitation Department is led by certified professionals in speech pathology, physical therapy, and occupational therapy. Our team develops personalized therapy plans to help our residents recover with the highest level of independence possible. Clinical services are available 24/7 and include: licensed nursing care, comprehensive wound care, pulmonary management, intravenous/enteral therapies, stabilization post-surgery, tracheostomy care, PICC lines, gastric tube care, NG tube care, TPN nutrition, and respite and hospice care.

Our short-term rehabilitation programs are customized to meet your individual needs. We simulate each patient’s home environment as accurately as possible in our ADL kitchen, transition suites, bathrooms, and laundry areas. Our fully-equipped therapy gym is designed with the latest treatment modalities. We offer a comprehensive range of services including endurance and mobility recovery, communication and cognitive augmentation, dysphasia management, orthotic fitting/training, pain management, community living, and activities of daily life.

We proudly offer Transition Rooms as part of our unique rehabilitative program, where residents can restore their cooking, bathing, and movement skills in a homelike environment. These fully-equipped private rooms help our staff ensure that residents are well-prepared for a successful and confident discharge.

The Pavilion at Creekwood supports not only the unique medical needs of our patients but also their distinct passions. We organize regular field trips into the community with private transportation, such as movie outings, restaurant visits, and shopping trips. We also have a monthly calendar packed with opportunities to socialize, participate in games, appreciate arts and music, join a religious service, enjoy pet visits, and more. We believe in a person-centered approach to the dining experience. Our dining menus uphold the philosophy of the American Dietetic Association and the American Diabetes Association. Every meal is prepared fresh and in accordance with our residents’ individual needs and preferences.
